How do I convert lb to gsm?

Multiply the pound weight by a factor that depends on the paper grade's basis size. Text stock uses ~1.48, cover ~2.71, bond ~3.76. The converter applies the right factor once you pick the grade.

Why does 80lb cover differ from 80lb text?

US pound weights are measured on different basis sheet sizes per grade, so the same number means very different thicknesses. 80lb cover (~216 gsm) is far heavier than 80lb text (~118 gsm) — always state the grade.

Is gsm the same worldwide?

Yes. Grams per square metre (gsm) is an absolute measure of weight per area, which is why it is unambiguous. US pound weights are relative to grade, which is why conversion needs the grade.
